About Us

Vivent Health + TPAN is a nonprofit health center in Chicago providing care for people living with or at risk for HIV. We use an integrated care model, meaning medical, behavioral, and social services work together to treat the whole person. Our services include primary medical care, mental health and substance use treatment, case management, HIV prevention, a food pantry, and an on-site pharmacy—designed to meet each client’s needs in a coordinated, seamless way.

Based in Edgewater, we also reach into nearby neighborhoods like Uptown and Rogers Park, where HIV rates are among the highest in the city. Our work centers on people most impacted by health disparities—those facing poverty, stigma, unstable housing, and other barriers to care.

We also operate the Tea Room, a safe, affirming drop-in space for LGBTQ+ young adults (ages 18–29), many of whom are experiencing homelessness. It offers community, basic needs like laundry and snacks, and access to care, housing support, and public benefits.

Nationally, we publish Positively Aware, an HIV treatment and advocacy magazine trusted by over 100,000 readers per issue—including individuals, clinics, and community organizations—sharing vital information and lifting up the voices of those most affected.

TPAN staff can often often be found at local health fairs. They play a crucial role in promoting wellness and providing essential health information to the community. They offer a variety of services, including free health screenings and educational maaterials.
